This should be similar to the Cattle Obs concept. I think we should also have a simple Yes/No button. Just not sure what the trigger level should be yet.

• Lightheaded

• Headache

• Dizzy

• Thirsty

• Weakness

• Irritable

• hear to breathe

• muscle cramp

• tiredness

• nauseous

• felt hot

• trouble concentrating

• “goosebumps” or chills

Choose symptoms and rate (Not at all, A little, Somewhat, Moderate, A lot, Extreme)
